Professor Alexander Margulis, the head of the Department of Plastic Surgery at the Hadassah clinic, takes on surgeries that the vast majority of plastic surgeons consider impossible. He has been practicing plastic surgery for over 20 years, with a total of more than 8,000 successful operations, of which more than 6,000 were performed on children.

Among the most common procedures conducted in the department are the correction of “cleft lip” and “cleft palate,” facial bone reconstruction after injuries and accidents, ear and nose reconstruction, removal of giant nevi on the face and body, and restoration of skin after severe burns. In fact, there is not a single, even the narrowest specialization in the field of pediatric plastic surgery that the “Hadassah” clinic cannot offer to patients.

The advantages of pediatric plastic surgery at Hadassah

The main reason why Professor Margulis succeeds in accomplishing tasks that other plastic surgeons rarely undertake, is primarily due to his experience and skill.

However, of course, part of the success is explained by the use of innovative techniques, such as an innovative method of stretching healthy epidermal tissue by inserting a special elastic silicone balloon underneath it, which is filled with saline solution. The skin stretches due to the created tension, and during the operation, the excess is used to cover the area after nevus removal.

This method allows for complete coverage of large defects and rarely causes rejection of the transplanted tissue. Currently, the doctors at the Hadassah clinic have the world’s largest experience in using this method. To avail this technique, dozens of adult and young patients from different countries around the world are brought to the clinic.

Some cases from the everyday practice

Stretching epidermal tissue was performed on a boy with a nevus covering the area from his waist to his left knee. The defect was completely removed during 4 surgeries, during the last of which two silicone balloons filled with saline solution were removed from under the child’s skin, and the excess skin completely covered the exposed areas created after the removal of the last fragments of the nevus.

Professor Margulis also successfully treats deep wounds and scars, which his colleagues from abroad rarely undertake.

One of the most talked-about stories in this field is the treatment of a four-year-old girl from Russia whose face was severely disfigured by an attack from a fighting dog. Over the course of nearly 5 years, Professor Margulis performed more than 8 surgeries, gradually restoring the girl’s face to a normal appearance. According to the professor’s estimates, the little patient will need 2-3 more surgeries. While her scars will remain for life, and she will have to recover from physical and psychological trauma for a long time, Professor Margulis is very optimistic about her quality of life and her ability to develop, learn, work, and build personal relationships in the future.

Diagnostics

Hadassah Hospital, located in Israel, is renowned for its excellence in pediatric plastic surgery. The hospital’s commitment to the highest standards of patient care extends to its meticulous diagnostics procedures, ensuring the safety and well-being of children undergoing plastic surgery.

Before any pediatric plastic surgery at Hadassah Hospital, a thorough and comprehensive preoperative evaluation is conducted. This evaluation involves a multidisciplinary team of experts, including pediatric surgeons, anesthesiologists, pediatricians, and nurses, who work together to assess the child’s overall health, medical history, and the specific surgical needs.

The diagnostic process begins with a detailed review of the child’s medical history. This includes any previous surgeries, allergies, chronic medical conditions, and medications. A comprehensive understanding of the child’s medical background is vital in tailoring the surgical approach and anesthesia plan to their unique needs.

A meticulous physical examination is performed to assess the child’s current health status. This examination helps identify any underlying medical issues that may need to be addressed before surgery. It also aids in evaluating the feasibility of the planned surgical procedure.

Depending on the nature of the surgery, various laboratory tests, such as blood work and diagnostic imaging (X-rays, MRI, CT scans), may be conducted. These tests provide crucial information about the child’s overall health and the condition of the surgical area.

“Cleft lip” and “cleft palate” are usually identified during ultrasound (sonogram) examinations. The more severe the defect, the earlier it can be detected. In the most severe cases, this can happen around the 13th to 14th week.

If only a “cleft lip” is concerned, it can be detected during an examination around the 20th to 24th week.

A “cleft palate” if it’s the sole defect, is rarely diagnosed during pregnancy because the facial bones conceal the palate of the fetus.

Checking the nuchal translucency of the fetus, performed around the 11th to 13th week, can indicate a risk of these defects forming, prompting more thorough diagnostics for the woman.

Pediatric plastic surgery often involves addressing congenital or acquired conditions that can impact a child’s self-esteem and emotional well-being. Hadassah Hospital recognizes the importance of psychosocial assessment to understand the child’s and their family’s emotional needs, offering support and counseling as necessary.

Hadassah Hospital prioritizes collaborative decision-making involving the child’s parents or guardians. The medical team discusses the surgical procedure, potential risks, expected outcomes, and any alternative treatments. Informed consent is obtained, ensuring that the child’s best interests are at the forefront of every decision.

The methods of treatment

Hadassah Hospital in Israel stands at the forefront of pediatric plastic surgery, employing a range of innovative methods to enhance the lives of children facing congenital or acquired deformities. With a team of highly skilled surgeons, state-of-the-art technology, and a commitment to holistic patient care, Hadassah Hospital’s approach to pediatric plastic surgery is both comprehensive and compassionate.

Giant nevi removal

Removal of a giant nevus is carried out in several stages with histological examination of the biomaterial.

Tissue expansion is commonly used for conditions like giant nevi or severe burns, resulting in improved aesthetics and function. At Hadassah Hospital, tissue expansion is employed to gradually stretch existing skin, creating more coverage for surgical reconstruction.

Craniofacial Surgery

One of the cornerstones of pediatric plastic surgery at Hadassah Hospital is craniofacial surgery. This specialized field addresses congenital and acquired abnormalities of the skull, face, and jaw. Using advanced imaging techniques like 3D CT scans, Hadassah’s surgeons can plan intricate procedures with unparalleled precision. From cleft lip and palate repairs to craniosynostosis corrections, these procedures not only improve a child’s physical appearance but also their ability to eat, speak, and breathe normally.

“Cleft lip” is a defect of the upper lip, often taking the form of a cleft, although it can be wider and involve the nose. It can appear on one side of the mouth, in the center, or on both sides.

“Cleft palate” is a cleft or opening in the palate. The defect can be located in either the posterior soft or anterior hard part of the palate, and it can even extend closer to the teeth, involving the upper jaw and teeth.

A team of experts, including a plastic surgeon, orthodontist, maxillofacial reconstruction specialist, speech therapist, and representatives from related fields, is responsible for comprehensive treatment.

Operations for “cleft lip” correction are performed at the “Hadassah” clinic from the age of 3 months. Correction of “cleft palate” is performed from the age of 10 months.

Microsurgery

Hadassah Hospital is a pioneer in pediatric microsurgery. This technique involves using high-powered microscopes and tiny instruments to repair or reconstruct delicate tissues. Microsurgery is especially valuable in cases of limb reattachment, complex wound closures, and the restoration of function after traumatic injuries. It allows for precise suturing of blood vessels and nerves, enhancing the chances of successful tissue grafting and functional recovery.

Endoscopic Procedures

Hadassah Hospital embraces minimally invasive techniques, including endoscopic procedures, for pediatric plastic surgery. These procedures involve small incisions and the use of a camera to access and treat certain conditions. For instance, endoscopic-assisted craniosynostosis correction minimizes scarring and accelerates recovery for infants with skull deformities.

Composite Tissue Allotransplantation

In rare and complex cases, where traditional reconstructive methods may not suffice, Hadassah Hospital has ventured into composite tissue allotransplantation (CTA). CTA involves transplanting a composite of tissues, such as face and hands, from a donor to a recipient. Hadassah’s skilled surgeons, working closely with immunologists, have achieved groundbreaking results in CTA, offering new hope to children with severe facial deformities or limb deficiencies.

Postoperative Care and Rehabilitation

The commitment to the well-being of young patients extends beyond the operating room. Hadassah Hospital has a dedicated team of pediatric specialists, including physiotherapists, speech therapists, and psychologists, who collaborate to provide comprehensive postoperative care and rehabilitation. This ensures that children not only recover physically but also emotionally and functionally.

Patient-Centered Approach

At the heart of Hadassah’s pediatric plastic surgery methods is a patient-centered approach. Children and their families are actively involved in the decision-making process, and the hospital’s multidisciplinary team provides thorough counseling to ensure that everyone is well-informed about the surgical procedure, potential risks, and expected outcomes.

Research and Innovation

Hadassah Hospital is committed to advancing the field of pediatric plastic surgery through research and innovation. The hospital collaborates with leading international institutions to explore new techniques, materials, and technologies that can benefit young patients. This commitment to continuous improvement ensures that children receive the most up-to-date and effective treatments available.
